When recovering from an illness or injury, there is no place like home to begin your road to recovery. We believe family, friends, and familiar home surroundings contribute to maintaining a positive outlook and often result in a more speedy recovery. Jordan offers a variety of home care services including nursing and therapy, as well as those who can assist with the most personal of needs such as helping with bathing, dressing, or preparation of meals.

What is home care?
Simply put, home care is healthcare delivered to you in your own home. Following an illness or injury, you may need continued care. Jordan’s clinical team members come to your home to provide the care you need. We also educate you on what steps you can take to remain independent, feel better, manage your disease, and have fewer hospitalizations.

How much does home care cost?
If you receive Traditional Medicare benefits your home care services are provided at no cost to you. If you subscribe to an HMO or have private insurance, you will be informed during the first visit what charges, if any, you will be responsible for.

What about my physician?
Every person’s home care needs are unique. Jordan’s nurses and therapists partner with you, your family, and your physician to determine what steps will be taken on your road to recovery. Together we will develop a plan to follow. As your needs change, the plan will change also.
